Treatment Reporting
We believe that a seamless line of communication between our practitioners and their patient’s referring physician is very important to our “Team” concept of patient treatment. One of the elements in this process is a notice of visit that our practitioners send to a referring physician whenever their patient visits our office.
This brief letter is simply meant to inform you that your patient has been to our office and for what reason so that you will always be aware of their progress and what services we have provided.
